Yes, you must pay $20 for the Transit Control Card and $100 entrance to the Galapagos National Park.
If you belong to a country from Mercosur or Comunidad Andina, your entrance fee to the Galapagos National Park is $50.
If you are Ecuadorean, your entrance fee to the Galapagos National Park is $6.
Depends if you have any particular condition. Quito is at 2800 meters above sea level. At the Cotopaxi National park the maximum altitude you might encounter if you decide to reach the first refuge parking lot is 4100 meters above sea level.
